General Summary of Position:
Provides primary nursing care and case management to for Limb Center inpatients/outpatients. Establishes protocols for streamlining in and out patient care. Plans and coordinates post-discharge and discharge care of the Limb Center patients. Prepares patient for outpatient status at the Limb Center. This position is 50% Nurse Practitioner and 50% Case Manager for inpatients. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital’s philosophy, policies, procedures, and standards.
EDUCATION/TRAINING: BSN from an accredited school of nursing. Successful completion of an approved Nurse Practitioner program.
EXPERIENCE: Three years of nursing experience, inclusive of at least one year of directly related experience in wound care and orthopaedics.
LICENSE/CERTIFICATION/REGISTRATION: State license in D.C. as a Registered Nurse and Nurse Practitioner. Credentialed as a member of the MGUH professional staff. CPR certification. Certification in ACLS and PALS preferred.
KNOWLDGE, SKILLS & ABILITIES: Knowledge of professional nursing theory, practice and medical care to give to and evaluate patients. Knowledge of organizational policies, regulations and procedures to administer patient care. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in taking medical histories to assess medical conditions and interpret findings. Ability to establish and maintain effective working relationships with patients, medical staff and the public. Ability to maintain quality control standards. Ability to remain calm and effective in emergency situations. Excellent interpersonal and communication skills.
Consideration will be given to an appropriate combination of education/training and experience.
Primary Duties and Responsibilities:
Assesses, facilitates and monitors the plan of care in conjunction with the patient and/or family/significant other.
Reviews all patient admission data within the designated time frames to determine the suitability of the level of care in accordance with established guidelines.
Identifies issues/problems and makes appropriate recommendations. Communicates with patients, families/significant others, medical staff, caregivers, and third-party payers, as necessary.
Coordinates care within MGUH and other settings by monitoring and overseeing the continuum of clinical care, ensuring necessary meetings are held with patient/family and the interdisciplinary team to coordinate treatment and discharge plans. Arranges discharge-planning referrals as ordered by patient’s physician.
Records and documents assessment data, interventions, results, and patient outcomes, per established clinical protocols.
Develops patient care plans and manages resources to optimize surgical results.
Instructs patients and family regarding medications and treatment instructions. Provides patient education and serves as resource to other clinical team members regarding particular expertise.
Triages patient telephone inquiries and provides consultation, as needed.
Collaborates with physician in evaluating and managing the acute and long-term medical needs of patients.
Assumes other duties and responsibilities that are appropriate to the position and area.
