General Summary of Position:
Provides medical management and care to patients in the Department of Medicine/Division of Palliative Medicine. Obtains complete medical histories and performs examinations on patients in the hospital and outpatient settings. Orders appropriate laboratory and diagnostic procedures.  Writes orders in charts and gives verbal telephone orders as needed. Reviews laboratory reports, radiographic reports, other diagnostic reports, and consultant reports. Provides telephone coverage for clinical problems called in by patients, nurses, caregivers, and others. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital’s philosophy, policies, procedures, and standards.
 
Qualifications:

  • EDUCATION/TRAINING: BSN from an accredited school of nursing. Successful completion of an approved Nurse Practitioner program.
  • EXPERIENCE: Three years of nursing experience, inclusive of at least one year of directly related experience in Internal Medicine. Home care experience preferred.
  • LICENSE/CERTIFICATION/REGISTRATION: State license in D.C., MD or VA as a Registered Nurse and Nurse Practitioner. Certified as an Adult or Family Nurse Practitioner. Credentialed as a member of the MGUH professional staff. BLS/CPR-certified.
  • KNOWLEDGE/SKILLS & ABILITIES: Knowledge of professional nursing theory, practice and medical care to give to and evaluate patients. Knowledge of organizational policies, regulations and procedures to administer patient care. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in taking medical histories to assess medical condition and interpret findings. Ability to establish and maintain effective working relationships with patients, medical staff and the public. Ability to maintain quality control standards. Ability to remain calm and effective in emergency situations. Excellent interpersonal and communication skills.
Consideration will be given to an appropriate combination of education/training and experience.

Primary Duties and Responsibilities:

  • Obtains complete medical history and physical data for hospital patients.
  • Performs physical examinations pertinent to the patient’s medical history and complaint.
  • Provides services related to health maintenance and promotion, palliative and end-of-life care.
  • Consults with palliative care physician or designees as needed; informs primary care physician of services provided and collaborates with another physician, if ordered; provides written reports to primary physician upon request.
  • Assesses acute medical problems on hospitalized patients.       
  • Orders and evaluates appropriate diagnostic studies and procedures.
  • Prescribes medications, including controlled substances to the extent delegated and licensed.
  • Interprets and synthesizes data to determine preliminary diagnosis and therapeutic plan and procedures.
  • Records and documents assessment data, interventions, results, and patient outcomes, per established clinical protocols.
  • Instructs patients and family regarding medications and treatment. Provides patient education and serves as a resource for other clinical team members.
  • Meets daily with the palliative care interdisciplinary team, including Doctors, Social Workers, Case Managers and Nurse Practitioners, to coordinate patient care and initiate discharge planning.
  • Makes daily rounds on patients with impending discharges to check that discharge paperwork is complete.
  • Makes follow-up telephone calls to patients to check on patient’s follow-up.  Patients are told before discharge to expect a phone call. Discusses with patient any problems or concerns post-discharge. We also ask how they felt about their stay in the hospital.
  • Instructs patients and family regarding medications and treatment instructions.
  • Collaborates with physician in evaluating and managing the acute and chronic medical needs of patient.
  • Performs other duties and responsibilities that are appropriate to the position and area.
