General Summary of Position:
Works in collaboration with a multidisciplinary healthcare team based in the department of Pulmonology/Critical Care, managing the sepsis patient population in the inpatient setting at MedStar Georgetown University Hospital (MGUH). Primary responsibility involves the coordination and triage of care for patients referred for sepsis evaluation. Specifically, this role requires preliminary assessment of the patient’s presenting clinical problem in the context of tests performed and clinical presentation toward determining the need for further diagnostic testing to be attained prior to proper triage and disposition of the patient for sepsis evaluation. In collaboration with the attending, the NP determines the need for any additional subspecialty physician consultation before final diagnosis and treatment is determined. Responsibilities include ensuring adherence to national clinical guidelines/protocols and ensuring an environment of safe quality care. The NP demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, and timely and compliant documentation. The NP is responsible for professional development and competency validation. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital’s philosophy, policies, procedures, and standards.
EDUCATION AND/OR TRAINING REQUIRED: BSN and MSN. from an accredited school of nursing. Successful completion of an approved Nurse Practitioner program.
LICENSURE AND/OR CERTIFICATION: State licensure in the District of Columbia as an Advanced Practice Registered Nurse and Nurse Practitioner certification with prescriptive privileges. BLS certification.
EXPERIENCE: Minimum of 1 year of critical care experience preferred.
SPECIAL KNOWLEDGE/SKILLS/ABILITIES: Knowledge of critical care hemodynamic monitoring and mechanical ventilation/respiratory physiology. Excellent interpersonal skills to communicate effectively with patients/families, as well as staff and physicians. Experience with arterial and central line placement and monitoring. Ability to react calmly and effectively in emergency situations. Knowledge of professional nursing theory, practice and medical care to give to and evaluate patients. Knowledge of organizational policies, regulations and procedures to administer patient care. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in taking medical histories to assess medical condition and interpret findings. Ability to establish and maintain effective working relationships with patients, medical staff and the public. Ability to maintain quality control standards. Ability to maintain calm and be effective in emergency situations. Excellent interpersonal and communication skills with a broad spectrum of professionals, including attending physicians, residents, medical students, trainees, support staff, and hospital management.
Consideration will be given to an appropriate combination of education/training and experience.
Primary Duties and Responsibilities:
Collaborates with interdisciplinary healthcare team in making diagnoses of acute and complex chronic conditions.
Interfaces directly with referring physician in the assessment of completeness and adequacy of outside results and other diagnostic testing, and suggests additional studies as needed to complement further evaluation and treatment by the attending physician.
May perform and document in the medical record a complete history and physical examination for acute and complex chronically ill patients with sepsis diseases. Assesses for risks associated with the care of the acute and complex chronically ill patient, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
Orders and collects data using appropriate assessment techniques relevant to supporting diagnostic information and diagnostic procedures where indicated. Collaborates in plan of care and follows assigned patient population.
Manages diagnostic tests through ordering, interpretation, performance, and supervision; formulates differential diagnoses by priority; diagnoses complications and orders appropriate interventions; identifies expected outcomes from diagnosis; and formulates and documents a plan of care to address complex acute and chronic healthcare needs.
Utilizes evidence-based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum.
Prescribes diagnostic strategies and therapeutic interventions, pharmacologic and non-pharmacologic, needed to achieve expected outcomes.
Educates patients regarding health promotion/illness prevention; instructs patient/family regarding medications and treatments; and facilitates communication with patient, family and staff to promote continuity of care across the outpatient continuum of care.
Demonstrates Professional Practice behaviors, including preceptor/mentoring, education and instruction of students, nursing staff, nurses, and graduate and novice nurse practitioners where appropriate,
Participates in educational opportunities, faculty and unit meetings, and M&M presentations. Maintains CEUs and membership in a professional organization.
Performs other duties and responsibilities that are appropriate to the position and area.
