General Summary of Position:
As part of an essential component of the MedStar Pituitary Center, this position is expected to provide internal and external coordination of care for a disease-specific subset of patients diagnosed, or suspicious for, pituitary adenoma. Navigation services are expected to provide individualized assistance to patient entering the healthcare system. This is done by helping to overcome barriers and facilitating timely access to quality care. Coordinates patient care among other multiple disciplines involved and the private practice community at large. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital's philosophy, policies, procedures, and standards.
EDUCATION/TRAINING: BSN required.
EXPERIENCE: Three years of professional clinical experience. At least one year of neurosurgery, endocrinology or outpatient experience highly desirable.
LICENSE/CERTIFICATION/REGISTRATION: Registered Nurse license in the District of Columbia. CPR certification preferred.
KNOWLEDGE, SKILLS & ABILITIES: Knowledge of medical equipment and instruments to administer patient care. Skill in applying and modifying the principles, methods and techniques to provide ongoing patient care. Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to react calmly and effectively in emergency situations.
Consideration will be given to an appropriate combination of education/training and experience.
Primary Duties and Responsibilities:
The main role of the navigator is to facilitate patient entry into the healthcare system to expedite care. Serves as the liaison between the patient and the healthcare team. Facilitates communication, assesses patient needs, identifies barriers to care, and implements solutions to lessen them. Advocates for patients and collaborates with the medical team to ensure timely progress toward the best clinical outcome and cost-effective care. Supports the patient's psychosocial needs by providing support, information and referrals as needed.
Facilitates access, patient evaluation and care planning through communication with the referring physician and the MedStar team. Stays current with the latest treatment modalities and services offered across MedStar.
Ensures timely appointments by working collaboratively with the scheduling team. Provides relevant financial information to preauthorization staff to screen for insurance requirements and eligibility. Identifies alternate referral sources if unable to accommodate at MGUH.
Follows all patients, ensuring that they are seen for routine long-term follow-up as required. Maintains a system for communicating appointments and reminders to patients, including completion of necessary lab work and imaging studies.
Maintains and reviews patient records. Obtains relevant radiographic images, pathology reports and other relevant reports performed at outside institutions for review prior to clinic visit.
Collects data through documentation of relevant information on the program's activities. Reports information to administrators and physician team.
Schedules in-services and education sessions with patients, families and healthcare professionals. Identifies communication barriers, financial concerns and cultural factors to overcome disparities in care.
Supports the research mission of the program by identifying opportunities for patient participation in clinical trials. Coordinates with the research team to promote enrollment. Provides information during patient inquiries re: clinical trials offered at the center.
Tracks and reports clinical outcomes and other performance data as required/requested to improve quality of care and clinical outcomes. Manages database to create reports.
Assumes other duties and responsibilities that are appropriate to the position and area.
