General Summary of Position:
As part of an essential component of cancer care services, this position is expected to provide professional nursing care for a disease-specific subset of oncology patients. Navigation services are expected to provide individualized assistance to patient entering the healthcare system. This is done by helping to overcome barriers and facilitating timely access to quality oncology care. Functions to advance the clinical mission and goals of MedStar Georgetown University Hospital and the Lombardi Comprehensive Cancer Center. This is done in collaboration with the divisions of General Surgery, Medical Oncology, Radiation Oncology, and Genetics Counseling, along with other disciplines within MGUH and the community at large. These functions are performed in accordance with all applicable laws and regulations and MedStar Georgetown University Hospital's philosophy, policies, procedures, and standards.

  • EXPERIENCE: Three years of professional clinical experience. At least 2 years of gastroenterology or outpatient procedure experience highly desirable.
  • LICENSE/CERTIFICATION/REGISTRATION:: Registered Nurse license in the District of Columbia. CPR certification preferred.
  • KNOWLEDGE, SKILLS & ABILITIES: Knowledge of professional nursing theory and practice to assess, plan, implement, and evaluate patient care. Knowledge of medical equipment and instruments to administer patient care. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to react calmly and effectively in emergency situations.

Consideration will be given to an appropriate combination of education/training and experience.

Primary Duties and Responsibilities:
  • Obtains a baseline of information used for the development and assessment of the individualized plan of care for each patient.
  • Works with an interdisciplinary team throughout the continuum of care to develop and manage the plan of care for the patient.
  • Assists patient/family with scheduling ancillary testing and other services and presenting necessary history, diagnostic/treatment studies and/or results, etc., throughout the continuum of care.
  • Plans for continuing care such as, but not limited to, patient and community services, community outreach resources and education resources.
  • Acts as a liaison between patients, families, the healthcare team, community resources, and other facilities to coordinate the provision of care.
  • Acts as a patient advocate to help identify and eliminate barriers to care.
  • Responsible for no-show rate report for procedures and clinic to ensure reduction of no-show rate by responding to all clinical questions and preparation instructions from patients.
  • Acts as a clearinghouse for incoming referral calls in an effort to streamline the delivery and coordination of care.
  • Ensures patients' referral process and transition into specialty services are timely and efficient; anticipates patient and family needs throughout the continuum of care.
  • Explores and connects patients with appropriate resources, health care and support services within MedStar Health; at other external facilities; and in their communities for timely diagnosis and treatment.
  • Supports and promotes quality and patient safety initiatives. Collaborates with physicians and associates to monitor and analyze departmental performance data, and identifies and participates in hospital and/or unit-based performance improvement activities including, but not limited to, core measures, nursing quality indicators and other patient care outcomes (e.g., procedure no-show rates and patient satisfaction).
  • Possesses working knowledge of physician/healthcare team members' plan of care, appropriate professional guidelines and standards of care. Contributes to development of internal clinical guidelines/pathways.
  • Able to work as a part of an interdisciplinary team through open, frequent and skilled verbal and written communication.
  • Facilitates the evaluation of patients by interfacing with referring physicians, clinical research coordinators, scheduling coordinators, consultants, insurance coordinators, and social workers. Has primary responsibility for the planning, implementation and evaluation of this process.
  • Coordinates the complex management and education of patients to ensure seamless integration with the healthcare team. Works closely with the scheduling team to ensure prompt facilitation of appointment(s) with providers.
  • Screens patients for appropriate information. Instructs patients in the collection of samples, completion of tests, and review of procedure preparation instructions. Performs patient telephone triage, assesses needs and coordinates needs with medical staff.
  • Maintains and reviews patient records, charts and other pertinent information. Posts test and examination results. Obtains and reviews consent forms, treatment plans and side effects as appropriate. Discusses potential complications with patient and family. Obtains relevant radiographic images performed at outside institutions for review prior to clinic visit and coordinates performance of relevant MedStar Georgetown radiographic procedures. Enters preliminary patient history information into electronic medical record in advance of physician visit.
  • Schedules in-services and education sessions with patients, families and healthcare professionals. Identifies communication barriers, financial concerns and cultural factors to overcome disparities in care.
  • Handles referral calls to ensure streamlined access to care. Ensures patient referral process for specialty services is coordinated, anticipating patient and family needs throughout the process.
