General Summary of Position:
Responsible for the coordination of patient care across the continuum of oncologic care for patients of the Blood and Marrow Transplant Program. This person will be responsible for providing assistance in coordinating patient evaluation; educating, informing and supporting patients and their families about treatment options; and facilitating treatment plans. This individual will function as a patient advocate, guiding patients and families throughout the bone marrow transplant care experience. These functions are performed in accordance with all applicable laws and regulations and Medstar Georgetown University Hospital's philosophy, policies, procedures, and standards.
Consideration will be given to an appropriate combination of education/training and experience.
Primary Duties and Responsibilities:
EDUCATION/TRAINING: Bachelor's degree in nursing required. Master's of Science in Nursing preferred.
EXPERIENCE: Demonstrates a high level of understanding of oncology treatment modalities and supportive therapies. 4-5 years of clinical nursing experience in oncology, minimum 3 years of bone marrow transplant experience required. Experience with medical oncology, inpatient/outpatient/community oncology services preferred. Must have clinical maturity and experience, excellent interpersonal skills and ability to work collaboratively with an interdisciplinary team of healthcare providers. Effective oral and written communication skills required.
LICENSE/CERTIFICATION/REGISTRATION: Licensure as a RN in the District of Columbia. Oncology nurse certification preferred.
KNOWLEDGE, SKILLS & ABILITIES: Position requires a high level of advocacy and problem-solving ability. Ability to multitask and organize advocacy and support for a large and varied group of cancer patients and their families.

Collaborates with physicians, nursing staff and multidisciplinary team in the assessment, planning, implementation, and evaluation of care for bone marrow transplant patients and their families. Functions as a clinical resource for nursing staff, physicians and multidisciplinary team in identifying problems and developing solutions to meet desired, measurable outcomes. Provides patient and family education in coordination with inpatient team specific to disease process and treatment. Primary point of contact for patients referred to and followed by the Blood and Marrow Transplant Program. Responds to referrals ASAP but not longer than 24 hours.
Coordinates all aspects of transplant care, coordinating apheresis and bone marrow collection services. Pre-transplant coordination with referring physicians. Admissions to hospital for transplant procedure. Discharge coordination in collaboration with inpatient transplant unit and case management team. This includes, but is not limited to, preliminary testing, scheduling and collaboration with the inpatient nursing team. Works with the interdisciplinary team throughout the continuum of oncology care to facilitate understanding of treatment modalities, diagnostic testing and treatment.
Facilitates weekly patient review meetings, presenting clinical and other relevant information to healthcare team regarding consults, pre-apheresis and post-apheresis, and transplant and post-transplant patient status. Functions as a liaison between patients, families, and healthcare team and community resources. Advocates for the patient by identifying and eliminating barriers to care.
Handles referral calls to ensure streamlined access to care. Coordinates the collection of pertinent patient information and manages the acquisition of necessary medical records for the initial appointment. Ensures that the patient referral process for specialty services is coordinated, anticipating patient and family needs throughout the process. Specialty services include, but are not limited to, surgery, medical oncology, inpatient services, rehabilitation services, home health, and psychosocial and spiritual care.
Monitors patient outcomes and participates in quality improvement activities. Contributes to the development of clinical guidelines, policies and procedures related to role.
Assists in coordination with research team members in the Clinical Research Management Office (CRMO).
Reviews laboratory results and diagnostic imaging, and implements follow-up plans in collaboration with the physician. Demonstrates competency to meet nursing care requirements for patients.
Maintains competency in all electronic systems required for job functioning.
Performs other duties and responsibilities that are appropriate to the position and area.
